7 Signs It’s Time to Stop Waiting and See a Fertility Specialist

I Know Why You Haven’t Come In Yet

You don’t want to “overreact.” You’ve been told to “just relax.” Everyone has a cousin who tried for two years and then conceived naturally. And honestly, walking into a fertility clinic feels like admitting something is wrong.

I hear this from almost every new patient I see. And I understand it completely.

But here’s what I’ve seen after years of practice: the couples who come in early get better outcomes. Not because we always need to intervene — sometimes I send people home and say “give it three more months.” But knowing what’s actually happening is never a bad thing.

Here are 7 signs that it’s genuinely time to come in.

1. You’ve Been Trying for 12 Months (6 If You’re Over 35)

This is the medical guideline, and it’s there for a reason. After 12 months of regular, unprotected intercourse without conception, the statistical probability of conceiving naturally in the next few months drops sharply. If you’re 35 or older, that window shortens to 6 months — because egg quality declines faster than most people realise.

2. Your Periods Are All Over the Place

I’m not talking about being a few days off here and there. I mean cycles that swing between 22 days and 47 days, or months where your period just doesn’t show up. Irregular cycles usually mean irregular ovulation — and without ovulation, conception isn’t possible that month. This is often PCOS, and it’s very treatable.

3. You’ve Had Two or More Miscarriages

One miscarriage is devastating but common — it happens in about 1 in 5 pregnancies. Two or more is called recurrent pregnancy loss, and it needs investigation. There are real, treatable causes — blood clotting disorders, uterine abnormalities, chromosomal issues — that we can find and address.

4. Your Periods Are Extremely Painful

Not “I need a paracetamol” painful. I mean “I can’t go to work for two days” painful. Severe dysmenorrhoea is one of the hallmark symptoms of endometriosis — a condition that quietly damages fertility over time and is often missed for years.

5. You Have PCOS, Thyroid Problems, or Diabetes

Don’t wait a full year if you already have a known condition affecting your hormones. These conditions directly impact ovulation and the uterine environment. Getting evaluated early means we can optimise your condition before treatment, which dramatically improves outcomes.

6. Your Husband’s Semen Analysis Wasn’t Normal

About 40% of infertility involves a male factor. If a semen analysis has shown low count, poor motility, or abnormal morphology — even mildly — come in together. There’s no point spending a year on female-only investigations when the answer might be right there.

7. Something Just Feels Off

I know this sounds vague, but I’ve had patients tell me they “just knew” something wasn’t right. Pelvic pain that isn’t period-related. Cycles that changed suddenly. A gut feeling. Trust that. A good evaluation either finds something useful or gives you peace of mind — both are valuable.

Coming In Isn’t Giving Up on Natural Conception

A fertility consultation doesn’t mean you’ll need IVF. It means you’ll have information. And information is power.

At Punit Fertility in Kandivali, Mumbai, the first consultation is a conversation — not a commitment to treatment.
